Immunizations
The immunization program at TriCounty Health Department offers vaccinations to people of all ages. TriCHD accepts private insurance and participates in the Vaccine For Children (VFC) program. VFC provides vaccines for children birth through 18 years of age who are enrolled in Medicaid, American Indian/Alaskan Native, under insured or uninsured. VFC vaccines are available free or at a reduced cost for eligible children.

What to bring to your immunization appointment:
1. Insurance card or information
2. Immunization records
3. A form of ID (Drivers License, Passport State ID, Student ID)
4. A form of payment (Visa, Discover, Mastercard or cash)
1. Contact your healthcare provider. Most healthcare providers in Utah are connected to the Utah Statewide Immunization Information System (USIIS). Contact your provider and ask if they can give you an Official USIIS Personal Utah Immunization Record. If your provider does not participate in USIIS, they can print your immunization record from their medical record system.
