The BeWise Program

TriCounty health Department is partnered with the Utah Department of Health & Human Services to offer the BeWise Program. The BeWise Program checks for heart disease risk factors and partners with other programs who screen for certain types of cancer. BeWise is for women ages 40-64 who are mid-to-low-income, underinsured, or uninsured.

The goal of BeWise is to provide women with the knowledge, skills, and opportunities to improve their diet, physical activity, and other lifestyle behaviors to prevent, delay, and control cardiovascular and other chronic diseases. When you participate in the BeWise program you may also receive education, goal-setting, and referrals to community-based programs that support physical activity, healthy eating, smoking cessation, and lower your health risks.


Eligibility

Free services for those who qualify include:
  • Blood pressure check
  • Blood cholesterol test (Lipid Panel)
  • Blood sugar test (fasting glucose or HbA1c)
  • Access to a personal health coach to help set and reach wellness goals
  • Referrals to other free health programs:
    • Chronic Disease Self-Management Program
    • Diabetes Prevention Program
    • Expanded Food and Nutrition Education Program (EFNEP)
    • Weight Watchers membership
  • Paid medical referral visit and follow-up care, if needed
